ANNEX 1 - MANDATORY CONDITIONS
No supply of alcohol may be made under this licence:
a) at a time when there is no designated premises supervisor in respect of it, or
b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence.

ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
Spirits will be located behind the counter area.
A digital CCTV system will operate and be maintained in line with current Home Office guidelines, recorded unedited images to be kept for 28 days and to be provided to Lincolnshire Police upon reasonable request.
All staff must be trained with regards to refusing sales of alcohol to underage persons and a record of training must be kept for inspection by the police.
Any persons attempting to purchase alcohol and appearing to be under the age of 21 must be challenged for acceptable photographic identification as accredited by Lincolnshire Police
Appropriate notices must be displayed conspicuously around the store regarding the sale of alcohol to underage persons and that there is a CCTV system in operation within the premises.
A refusals book must be kept and maintained. The details should include date and time of attempted transaction, the individuals name or description, the product that the individual attempted to purchase and the staff member’s name who dealt with the individual.
