ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. The Premises Licence Holder shall give 6 months written notice to Lincoln Police Licensing Office, Lincoln Police Events Planning Officer, the Event Planning Manager for Lincs Emergency Planning and the Fire Safety Officer for Lincoln Fire and Rescue, of any event, where it is intended or can be reasonably expected, that more than 499 persons will attend the event. This notice will enable a decision as to the necessity for a Safety Advisory Group for the event.
2. Such notice shall also be given to the Responsible Authorities (section 13(4) Licensing Act 2003)
3. The SAG or its equivalent, shall receive the finalised safety plan, completed to an acceptable standard in accordance with the Purple Guide or its equivalent document, at least 14 days in advance of each event.
4. There shall be provided at the Premises, door supervisors to such a number as the management of the premises considers are sufficient to control the entry of persons to the Premises, and for the keeping of order in the Premises when they are used for the provision of a licensable activity.
5. A record/log book shall be kept on the Premises by the Premises Licence Holder of every person employed on the Premises as a door supervisor.
6. The record shall contain the following details:
a) The door supervisors name
b) His/her Security Industry Authority Licence Number
c) The time & date he/she commenced and concluded their duty
d) The door supervisors shall sign each entry
e) The Premises Licence Holder or other authorised person shall also endorse each entry as having checked the authenticity of the individual door supervisor.
7. That record shall be available for inspection, on demand, by an Authorised Officer of the Council, the Security Industry Authority or a Police Constable.
8. Each entry shall be retained for a period of 24 months from the date of completion.
9. Each door supervisors licence number will be verified by the Premises Licence Holder or their agent using the internet SIA website, on the occasion of each door supervisors’ initial employment at the premises, and thereafter on each occasion that the door supervisors are used.
10. The verification check will be recorded and signed appropriately by the Premises Licence Holder or their agent.
11. Prior to any event appropriate staff training will take place to establish personnel roles and responsibilities with the object of creating a safe and comfortable environment for the proposed activity to take place.
12. Emergency lighting shall be supplied if the Premises Licence holder feels that this is necessary.
13. Emergency exits will be clearly marked and provision made for the safe entry and exit of any Emergency Vehicles. These exits will be regularly checked.
14. The Premises Licence holder or their agent shall carry out regular assessments of any noise emanating from the Premises.
15. An adequate number of waste receptacles shall be provided, and it shall be the responsibility of the organisers to empty and dispose of collected refuse at a frequency to be agreed with the Licensing Authority.
16. A Criminal Records Bureau (CRB) check shall be carried out for all personnel who are responsible for working closely with children, as laid down in relevant Child Protection Legislation.
17. There shall be no adult entertainment to take place at the Premises.
