ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
There shall be in place for the premises a written policy agreed with Lincolnshire Police, to prevent the sale or supply of alcohol to persons under 18 years of age.
That policy shall require any person who appears to be under the age of 18 to produce one of the following forms of identification only:
· Recognised proof of age card accredited under the British Retail Consortiums Proof of Age Scheme (PASS) and bearing the authentic hologram.
· Photo British driving licence.
· Passport.
· Any other means of identification approved and accredited in writing by Lincoln Police Licensing Office.
All point of sale staff shall undergo training in the above policy with a record kept of the date of training, signed by the member of staff and the trainer and retained for two years.
A notice or notices shall be displayed in and at the entrance to the premises where they can be clearly seen and read and shall indicate:
· That it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
· That proof of age may be required at the premises and those accepted means of proof of age listed as outlined above.
The Challenge 21 scheme shall be adopted.
A refusal register shall be maintained on the premises in the following format:
· The register to be bound in page format with consecutively numbered pages.
· Each entry to be timed dated and signed by the staff member making the refusal.
· Any CCTV evidence shall be retained for 28 days and cross referenced to the specific refusal.
· Name or description of person refused.
· Each entry to be signed by the Designated Premises Supervisor / Manager / Supervisor.
· The item/s that the person refused attempted to purchase shall be recorded.
· The reason for refusal shall be recorded.
The register shall be retained for 12 months and produced for inspection upon request of an officer of the Licensing Authority, Lincolnshire Police or Lincolnshire Trading Standards.
