ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
Children must be supervised by a responsible adult at all times whilst on the premises including times when using the toilet facilities.
Membership of any local pub watch scheme (where one exists) is required and would include registration.
A CCTV system shall be installed on the premises.
The system and equipment installed must be approved by the Lincolnshire Police following completion of a survey by a qualified Crime Reduction Officer.
The system must operate during the whole time the premises are open to the public. Leaving the equipment in operation during non-opening hours is also a good crime reduction initiative in the event of a burglary occurring.
The system must include a thirty-one day video library. Each tape must be endorsed with times and dates of recordings and shall be retained not less than 28 days before re-use.
The system must be capable of recording a satisfactory image from each and every camera continually eg. Multiplex.
The system must be capable of monitoring and recording entrances and exits, and any areas not easily observed by staff, including toilet areas.
The Licensee shall allow officers of the Lincolnshire Police/Local Authority access to the system at any time when the premises are open or at any other reasonable time.
The Licensee shall provide such tapes as requested by an officer of the Lincolnshire Police/Local Authority
There shall be provided at the premises door supervisors who are registered with the Security Industry Authority (SIA) to such a number as the management of the premises consider are sufficient to control the entry of persons to the premises and for the keeping of order in the premises when they are used for a licensable activity after midnight until closure.
The licence holder or his representative shall conduct regular assessments of the noise coming from the premises on every occasion the premises are used for regulated entertainment and shall take steps to reduce the level of noise where it is likely to cause a disturbance to local residents.
The licensee or responsible person shall ensure that no noise shall emanate from the licensed premises nor vibration be transmitted through the structure of the premises which gives rise to a nuisance or unreasonable disturbance to the occupiers of premises in the vicinity of the licensed premises.
Requests made by environmental health officers of the Council shall be complied with as soon as practicable. In addition, any restriction specified in the schedule to the licence regarding control to be exercised over music shall be implemented by the licensee or responsible person at all times when the premises are open to the public.
No glasses or bottles to be removed from the premises
There shall be in place for the Premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 25 years to produce a form of identification, which is compliant with the following:
1. The card has a verifiable means of accreditation for the information held upon it. (i.e. issued by Central or Local Government, recommended by the Home Office or ACPO, supported by Government e.g. Photo Driving Licence; Passport; PASS Card).
2. The information on the card provides the date of birth of the holder.
3. The card provides information to identify the holder both by name and photograph.
4. The card can be authenticated beyond reasonable doubt to avoid acceptance of fake/fraudulent identification.
A notice or notices shall be displayed in and at the entrance to the Premises where they can be clearly seen and read and shall indicate:
1. That it is unlawful for persons under 18 to purchase alcohol.
2. That it is unlawful for any person to purchase alcohol on behalf of a person under 18 years of age (unless the recipient is 16 or 17 years of age, is taking a table meal accompanied by an adult and consumes only beer, wine or cider with a meal).
3. That proof of age may be requested at the Premises and those accepted means for proof of age are as listed above.
All point of sale staff shall undergo training in the above policy with a record kept of the date of training, signed by the member of staff and the trainer.
Each entry shall be retained for a period of 24 months from date of completion.
