ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
Notices will be displayed at the entrances to the premises where they can be clearly seen and read and will indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person less than 18 years of age.
A CCTV system is installed and shall be maintained in working order. The CCTV recordings shall be retained under secure arrangements for a minimum of 21 days. Such recordings shall be produced to Lincolnshire Police or their nominated representative on request.
A refusals register will be maintained and include all incidents where a sale of alcohol is refused. The register shall contain the following information:
Time
Date
Reason for refusal
Details of the person (if available)
Description of the person
All entries shall be signed by the DPS.
All staff will be trained in the store policy for retail sales of alcohol. Training records will be maintained and signed by both the member of staff and the DPS. Records will be retained for 24 months.
There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. The policy shall require any person who appears to be under the age of 25 to produce a form of identification, which is compliant with the following:
The card has a verifiable means of accreditation for the information held upon it, (i.e. issued by Central or Local Government, recommended by Home Office or ACPO, supported by Government e.g. Photo Driving Licence; Passport; Pass card).
The information of the card provides the date of birth of the holder.
The card provides information to identify the holder both by name and photograph.
The card can be authenticated beyond reasonable doubt to avoid the acceptance of take/fraudulent identification.
