• ANNEX 1 - MANDATORY CONDITIONS

   

  1.              No supply of alcohol may be made under this licence:

   

  a)  at a time when there is no designated premises supervisor in respect of it, or

   

  b)  at a time when the designated premises supervisor does not hold a personal licence,

      or his personal licence is suspended.

   

  2.              Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence

   

  3.              The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.

   

  The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.

• ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE

   

  All staff will be trained in the legal requirements relating to the sale of alcohol and will receive refresher training on a 6 monthly basis.  Records of training will be retained, with each individual record signed by the trainer and staff member.

   

  A CCTV system will be installed and operated at the Premises during the times that the Premises is providing Licensable Activites.  All staff will be trained in the operation of the CCTV.

   

  Images will be retained for a period of 31 days and will be made available on request to Lincolnshire Police or Local Authority Licensing Officer.

   

  The Premises will operate a challenge 25 age policy in that any persons wishing to purchase alcohol who appear to be under the age of 25 years will be required to provide one of the following forms of photographic identification:

                        Passport

                        Driving Licence

                        Proof of age Card with accredited PASS Logo

   

  Only members of staff who are over the age of 18 years will be able to sell alcohol to customers.

   

  Any refusal to sell alcohol will be recorded in a Refusals Register which will be made available for inspection, on request, by Lincolnshire Police or Local Authority Licensing Officers
