ANNEX 1 - MANDATORY CONDITIONS

No supply of alcohol may be made under this licence:

a) at a time when there is no designated premises supervisor in respect of it, or

b) at a time when the designated premises supervisor does not hold a personal licence, or his personal licence is suspended.

Every retail sale or supply of alcohol made under this licence must be made or authorised by a person who holds a personal licence

Section 19 Licensing Act 2003

The following condition shall come into force on 1st October 2010

4. (1) The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.

(2) The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.

ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE

A tamperproof CCTV system shall be installed to the ground floor, maintained in working order and operated at the premises (in liaison with and to the satisfaction of Lincolnshire Police) and shall be used to record during all hours the premises are open. The recording shall be on removeable medium for which a secure storage system shall be provided. The images recorded by the CCTV shall be retained in unedited format for a period of not less than 31 days. All CCTV recordings shall be made available to Lincolnshire Police on request. The shop staff employed at the store will monitor the CCTV system at all times.

Notices will be displayed at the entrances to the premises where they can be clearly seen and read and will indicate that it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person less than 18 years of age.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY

There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. That policy shall require any person who appears to be under the age of 18 years to produce one of the following forms of identification:

a.              A recognised proof of age card accredited under the British Retail Consortiums Proof of Age Standards Scheme (PASS).
b.              Photo driving licence.
c.              Passport.

All staff shall undergo training with regarad to the written policy and the prevention of the sale of alcohol to minors, with a record kept of the date of training, signed by the member of staff and the trainer. Each member of staff shall also sign an acceptance of the Policy.

Each entry shall be retained for a period of 24 months from date of completion.

The licence holder shall ensure that the area set aside for the display of alcohol shall be restricted to the public outside the licensed hours.
